How do I apply for a Forest Practices Application, Notification, or Renewal?
You can get applications, maps, and other forms on the Forest Practices Forms & Instructions page or at the DNR region office which serves the area where your forest land is located. The Forest Practices Application/Notification also has a set of instructions that is beneficial to look through while filling out the FPA/N.
